At CareLinks, we share in that concern, and we are here to help keep you on a course to improved health.
Current trends in healthcare indicate a teambased approach to care reduces the risk for being readmitted to the hospital within 30 days of discharge. Hospitals know they need to do a better job helping patients immediately after they leave the hospital. CareLinks is here to do exactly that.

The time immediately following your stay at the hospital is a crucial point of needing highly specialized care. In addition, you are also at the highest risk of being readmitted to the hospital. CareLinks is here for you during this time to help you bridge that care gap. We provide you a convenient location to receive a quick "check up" to keep your condition from worsening. We do not replace your primary care provider but rather serve as a link between you and your doctor at a critical time in your care.
A highly skilled practitioner will be assigned to your case and play a central role in ensuring your care is on track following hospitalization. You will be provided with a visit summary for easy coordination of care. The practitioner will then communicate with your primary care physician and take the necessary steps to make your follow-up care successful.
Your next appointment at CareLinks will be written on the back of the brochure for your convenience. Please keep your appointment and bring all medications and paperwork that you received at discharge from the hospital or emergency room. CareLinks can even prescribe medications, order tests, and perform other procedures, if necessary.

CareLinks is open Monday through Friday, 7:30a.m. to 4:00p.m. If you are calling CareLinks to provide a ride home for discharged patients, we can accept patients up until 3:30p.m.
