Helping Nursing Facilities Keep Residents Healthier
Advanced elderly patients constitute one of the fastest-growing segments of our population. As this older population continues to rise, so does the need for high-quality, extended-care services beyond hospital and clinic walls.
Franciscan Health recognizes the need for consistent, quality healthcare within long-term care facilities. In Northern Indiana, we have extended our care to area nursing homes through the "snificists" (skilled nursing facility physicians) program in an effort to keep geriatric residents healthier and to better treat and shorten the stays of those needing admission.
Snificist program benefits
- Physicians and nurse practitioners who regularly visit the skilled nursing facilities provide residents consistent, high-quality care - equal to that found in the hospital.
- Physicians who round in nursing facilities work with hospitalists (hospital-based doctors) and specialists so information is more effectively shared.
- Nursing home patients are being seen by physicians who specialize in care to the elderly.
- Nursing home standards of care increase along with the individualized attention residents receive.
