Telehealth: Convenient Care from the Comfort of Your Home
Telehealth is a web-based program designed to allow your care providers to monitor your status and provide you with more convenient healthcare from the comfort of your home. This service is available for patients currently under home healthcare services.
The goal of our telehealth program is to decrease hospitalizations by identifying and treating changes. This program does not replace the nurse or doctor. It provides additional information to assist with your care.
Telehealth patients take home equipment to monitor a variety of vital signs, including blood pressure, oxygen saturation, pulse rate and weight (up to 450 lbs.). A wireless gateway unit collects the information and transmits the vital signs safely and securely to our care team.
Is telehealth right for me?
Possible candidates for Telehealth include patients with:
- Chronic conditions including congestive heart failure, diabetes mellitus and chronic obstructive pulmonary disease
- Multiple emergency room visits
- Frequent hospitalizations
- Palliative care needs due to a terminal illness
- Complex medication regimens
For patients with chronic conditions, even small changes can add up to large concerns. Monitoring your vital signs on a regular basis allows our medical professionals to respond quickly when a concern is detected. In addition, our care team analyzes the daily information and works with your doctor to adjust medication and nutrition regimens to successfully manage your chronic condition.
