Franciscan Health Lafayette's Inpatient Rehabilitation Prepares for Move to East Campus
Franciscan Health is proud to offer comprehensive inpatient rehabilitation that offers the continuum of care for many patients in Lafayette.
The inpatient unit has a rich history, dating back to 1994. We first opened our doors at Home Hospital in January of 1994. In January of 2010, we moved our location to the central campus of Franciscan Health Lafayette (formerly known as St. Elizabeth Central). And now we are excited to announce our new location will open in April 2018 in the West Tower at Franciscan Health Lafayette East.
Our inpatient unit is a general rehabilitation center that cares for a variety of disabilities and illnesses. Pradeep Gnanapragasm, MD, a physiatriast, is the medical director for our inpatient unit. Dr. Pradeep works with patients and multidisciplinary teams to develop an individualized plan of care with intensive functional interventions and treatments. The goal for the patient is always to regain as much independence as possible. Members of this team include registered nurses, rehab case coordinator, physical therapists, occupational therapists, speech-language pathologists, social work, respiratory therapists and spiritual care.
We are certified by CMS (Centers for Medicare and Medicaid Services) and follow their criteria for patients to be admitted to inpatient rehabilitation, which includes:
- Patient must be able to participate and benefit from three hours of therapy a day, five days a week. All therapy is done one-on-one
- Patient must have the need for a minimum of two different types of therapy. The combinations could be a physical and occupational therapist or physical and speech-language pathologist.
- Patients must have the need for close medical supervision by the physiatrist, the need for three face-to-face visits per week.
Dr. Pradeep and rehab nurses work closely together to provide the highest quality of care for each patient. Nurses work closely with patient care assistants to ensure the therapy training is implemented 24 hours a day. The rehab nurses also work very closely with the therapists to help achieve patient's goals towards their discharge.
