Franciscan Health Pharmacy Team In Hammond Recognized For Patient Safety
HAMMOND, Indiana - Franciscan Health Hammond’s Comprehensive Transitions of Care (TOC) Pharmacy Program was recently recognized by the Indiana Hospital Association (IHA) as winners of the 2018 Patient Safety Innovation Award at this year's Indiana Patient Safety Summit.
Sun Lee-Such, Administrative Director of Pharmacy Services Northern Indiana & South Suburban Chicago Divisions, (pictured, front row center) presented the success of the Comprehensive TOC Pharmacy Program to over 300 health professionals and accepted the award on the team's behalf.
The Comprehensive TOC Pharmacy Program has bridged gaps in the transition of care from the hospital to the home and decreased readmissions of high-risk patients. Through integration of Transition of Care Pharmacists, Medication Reconciliation Pharmacy Technicians, Pharmacy Patient Assistant Coordinators, ambulatory Pharmacotherapy Clinic, and Franciscan Health Hammond's Readmission Committee, the program's unique approach removes patient care silos by improving continuity of care in all aspects of a patient's healthcare experience.
