CRAWFORDSVILLE, Indiana - The Franciscan Healthy Living Center has opened an office at 1704 Lafayette Road, Suite 4, in Crawfordsville in the Sugar Creek Medical Center on the Franciscan Health Crawfordsville campus.
"Adding this service to our community is critical to preventative care management and ensuring our patients have complete care," says Terry Klein, chief operating officer of Franciscan Health Crawfordsville. "I am proud that Franciscan Health will be able to offer these services to the Montgomery County community."
The Franciscan Healthy Living Center Crawfordsville brings together medical professionals dedicated to the prevention and treatment of chronic healthcare conditions. The center will offer individual appointments for diabetes self-management education. The center will add services for heart failure, smoking cessation and COPD management in the future.
"Our goal is to provide education and tools to assist with improving quality of life and reducing healthcare complications," says Jessica Corbin, RN, director of Franciscan Healthy Living Center.
Cindy Cosenza, RD, CDE, is now accepting appointments for diabetes self-management education based on a referral from a healthcare provider. Patients will meet one-on-one with Cindy at the Sugar Creek Medical Center. For more information or to schedule an appointment, please call (765) 364-3120.
Diabetes is a common chronic disease which may result in a variety of serious health conditions. The highly trained education staff at Franciscan Health Crawfordsville offers compassionate support and encourages patients to successfully manage their diabetes. Managing diabetes well help reduce the risk of diabetes related health problems.
The Franciscan Healthy Living Center, in conjunction with Franciscan Health Crawfordsville, offers a monthly diabetes education series and support group. The next meeting is scheduled on Tuesday, May 15, 2018. The group will meet from 3 to 4 p.m. in Bonzel Hall, located inside Franciscan Health Crawfordsville.
