Pharmacy Practice Residency (PGY1) in Dyer and Hammond

The Pharmacy Department at Franciscan Health Dyer & Hammond is one of the most progressive in Indiana with a wide variety of clinical pharmacy programs utilizing a large number of pharmacy-driven protocols on a daily basis. Our PGY1 Residency program is to provide exemplary training and mentorship preparing first year pharmacy residents for general direct patient care positions or for PGY2 training in area of choice.

How To Apply

PGY-1 Requirements

Resident candidates must be graduates or candidates for graduation from an ACPE-accredited degree program as well as be a licensed pharmacist or eligible for licensure in the state of Indiana. The candidate must be a citizen or legal resident of the United States. 


Resident candidates must participate and adhere to the rules of the Residency Match Program and submit all materials through PhORCAS. Application deadline is Jan. 8, 2021. 

The following items are required for a complete application: 

  • Letter of interest
  • College transcripts
  • Letter or recommendations (ASHP online format). Total of three required
  • Curriculum vitae
