BROOK, Indiana - Franciscan Brook Health Center and Wheatfield Health Center will be offering sports physicals for students this summer for $35, payable by check or credit card.

Sports physicals will be offered from Tuesday, July 9 to Tuesday, July 16, 2024, from 8 a.m. to 4 p.m. Central each day.

Immunization records and any required physicals forms should be brought to the appointment. Immunizations will not be administered at the appointment.
