WHEATFIELD, Indiana - Franciscan Wheatfield Health Center will be offering sports physicals for students this summer on Wednesday, June 11, 2025, Wednesday, June 25, 2025 and Tuesday, July 22, 2025, from 8 a.m. to 4 p.m. Central each day.

Immunization records and any required physicals forms should be brought to the appointment. Immunizations will not be administered at the appointment.

Physicals are $35, payable by check or credit card.
