Our Approach
Your heart, kidneys, liver and metabolic system do not function independently. Changes in one can affect the others and contribute to conditions such as cardiovascular disease, chronic kidney disease, fatty liver, obesity and type 2 diabetes.
The American Heart Association formally defined cardio-kidney-metabolic (CKM) syndrome to recognize these important connections and provided a framework for preventing and managing disease across multiple organ systems.
At Franciscan Health, our cardiometabolic team of specialists evaluate your complete health story. This may include checking your blood pressure, cholesterol, blood sugar, kidney function, liver, weight and other health factors to identify risks and develop a personalized treatment plan.
Depending on your needs, care may consist of lifestyle changes, medications, further testing and ongoing management of existing conditions. By addressing these connected, related risk factors, our providers can help you better understand and manage your cardiovascular, kidney, liver and metabolic health.
What Is Cardiometabolic Syndrome?
Cardiometabolic syndrome is a health disorder involving the connections among cardiovascular disease, kidney disease, fatty liver, obesity and diabetes. Because these conditions can affect one another, having risk factors or disease in one area may increase the likelihood of developing health problems in another.
Cardiometabolic syndrome includes a range of affected people, from those with risk factors but no cardiovascular disease to those who already have cardiovascular disease. Identifying where you are on this spectrum can help your healthcare team determine your appropriate course of treatment.
Who Should See A Cardiometabolic Specialist?
Your starting point for cardiometabolic care depends on a few factors:
If you have diagnosed cardiovascular disease: Request an appointment with a Franciscan Health cardiologist who specializes in cardiometabolic health. Your provider will go over your next steps and evaluate how your heart health may be affected by kidney disease, diabetes, and other metabolic risk factors.
If you do not have diagnosed heart disease:Schedule an appointment with a primary care provider who specializes in cardiometabolic health. Your provider will check all your health numbers and go over your next steps for prevention, testing or treatment.
Most people with cardiometabolic disease are unaware and do not have symptoms.
What are Cardiometabolic Risk Factors?
Your provider will determine your cardiometabolic risk by checking for:
- High blood pressure
- High cholesterol or triglycerides
- High blood sugar or prediabetes
- Type 2 diabetes
- Overweight or obesity
- Chronic kidney disease
- Existing cardiovascular disease
- Liver function
